The Secret Boss fight of Persona 5 is against Caroline and Justine. This fight cannot be done on your first playthrough and can only be fought on New Game Plus. In order to initiate it, go to Mementos and talk to Caroline next to the Velvet Room on the top floor. Even though the fight can be done as early as May 9th, it's not recommended to attempt it then because you'll be severely underleveled. Don't attempt this fight until you're leveled up to at least Level 60. Also, make sure to save beforehand. If you lose, you'll lose all of the healing items you had used and you'll be left with 1 HP.

Be prepared by having with you multiple high level personas that either Nullify, Drain, or Repel one of each attribute. If the twins manage to knock down every one of your party members, they'll do an All Out Attack which instantly kills you. Fusing the Persona Yosh*tune is a great idea since it can be immune to all attacks. Get the max Rank for the Death and Star Confidants before entering the battle. Takemi's shop will sell Magic Ointments when you have a high confidant rank with her which helps out a lot by reflecting magical attacks your party member is weak to. Buy a lot of Takemedics so that you don't use up too much SP. Maxing out Star lets you switch out your party members without waiting for Joker's turn in order to avoid attacks a party member is weak to. Also, try to have a bunch of Coffee in your inventory since you'll be using ALOT of SP during this fight.

There are 2 important things to remember for this fight:

The first is that you must do a specific amount of damage to the two of them (1350 individual or 2700 total damage) after a set amount of turns pass for each phase. The First phase lets you go for 6 turns while every other phase has 5 turns. If you don't do enough damage by the turn limit, they'll kill you instantly.

The second is that you need to kill both of them during the same turn. If one is dead while the other one isn't when their turn comes up, the alive one will revive the dead one with Half of their HP (4000). So it's important to make sure that you do the same amount of damage to them with attacks that target both of them at once. Only use single target attacks when one has a lot more HP than the other.

Phase 1

The first phase will last for at least 6 total turns. Caroline and Justine will only use these two attacks.

Make sure to have a Persona with resistance to physical and/or gun attacks in order to avoid taking too much damage. Heal with takemedic when you need to. If they knock somebody down; they'll either remove their stat debuffs, remove your stat buffs, or launch more attacks hoping to knock you down.

It's wise to buff your party while debuffing all of Caroline and Justine's stats given how strong they are and how frequently they dodge attacks. So lower their Attack, Defense, and Agility/Evasion. Use your strongest group attacks that target both of them for damage. It's important to keep doing damage because if you don't do around 2700 damage to them by the time the 6 turns are up, they'll automatically kill you.

Phase 2

After 6 turns pass, they'll speak to you and if you did enough damage, the fight will continue. They'll change up their moveset by alternating between 4 new attacks in this order.

This is when they start singling out your party members to knock them down with the elements they're weak to. They'll usually use the group version of the attack except when they want to target a party member that's weak to the element. The two of them never shake from this pattern so make sure Joker's Persona won't be weak to their attacks for their next turn. If a party member is too vulnerable for you, either switch it out or have them guard when the attack they're weak to is about to occur. You'll have 5 turns to do around 2700 damage again so keep your buffs on and their debuffs on as well. If you did enough damage after 5 turns, the next phase will commence.

Phase 3

Caroline and Justine will once again switch up their attacks with 2 new attacks for each of them. They'll use them in this order.

Ma-Kougaon Ma-Eigaon

The good news is that nobody in your party should be weak to Bless or Curse damage unless Joker's equipped Persona is. By now, you should have depleted half of their health. When that happens, they'll fully heal each other. Don't panic because this was always going to happen and they won't do it again. When you do around 2700 damage again, the next phase will commence.

Phase 4

For the next 5 turns, Caroline and Justine will use their previous attacks in the same order they were first used. You're likely running low on SP now so use SP recovery items like Coffee or even a Soma to gain it back. Don't worry about wasting a Soma because you'll be reimbursed if you win. Keep up with buffing and debuffing while making sure to do another 2700 damage in 5 turns. After that's complete, the next phase will commence.

Phase 5

Caroline and Justine will be doing a lot more damage with their attacks from now on so be prepared to heal with items to save SP. Their next attacks are in this order:

Thunder Reign/ Wild Thunder Panta Rei/ Vacuum Wave
Psycho Force/ Psycho Blast Atomic Flare/ Cosmic Flare
Divine Judgement Demonic Decree

They'll be going over every type of damage during their 5 turns so be prepared to either guard or switch out party members weak to their upcoming attacks.

You'll have a golden opportunity to do massive damage to the two of them in this phase. After Justine does her Nuclear damage attack, have the entire party use Magic Ointment on themselves. Their next two attacks are Divine Judgement and Demonic Decree which are Bless and Curse attacks that slashes your total health in half. With the entire party using Magic Ointment, the attacks will be reflected back at them and they'll lose half of their health instead. If you want to avoid the chance of them attacking the same party member again without their Magic Ointment on, equip Joker with a Persona that repels both Bless and Curse skills (preferably Odin or Yosh*tune) and use a Limelight on him. That way they'll specifically target Joker when doing their attacks. But it will last a few turns longer so equip Yosh*tune to not take so much damage from their attacks.

When you've done 2700 damage, the 6th and final phase will commence.

Phase 6

For the Final Phase, they'll use the same attacks in the 5th phase. Except this time, they'll be using them in a random order. Make sure that you've increased your party's defense in order to survive the final phase since you don't want to lose at this point.

Above everything else, make sure their remaining health is close together as possible. If one dies and the other one is still alive after your turn is over, the remaining twin will revive the other twin with 4,000 HP forcing you to battle for much longer. So make sure that they die within the same turn.

You don't have to worry about doing a specific amount of damage in the last phase. But if you don't complete the battle within 50 turns, they'll automatically kill you. [1]
